Community Link Worker Project Annual Review

SVE, alongside our sister organisation, CTSI, manage the Community Link Worker (CLW) Programme in Clackmannanshire and Stirlingshire. 

The CLW programme supports the Health and Social Care Partnership’s prevention agenda and has developed social prescribing in the Partnership area. The programme offers support to people who attend their GP and sometimes require support with social or other issues. The CLWs provide 1:1 support to people to enable them to identify areas which are negatively impacting on them and support them to improve their health and wellbeing and enable people to self-manage health conditions by providing information and support and signposting to appropriate services.

CLWs have links with other local organisations such as CAB, Carers Centre and Stirling Council on Disability and Third sector organisations in Clackmannanshire and Stirling who provide activities which support people to maintain and improve their health and wellbeing such as mental health service providers, walking groups, community activity groups.

People can be referred to the service by their GP, other health worker or self-refer. Ask at reception to make an appointment.
